Survey Instructions:



      Log in

  • Log in using your user name and temporary password provided by the State.
  • During initial log in, you will be prompted to change your temporary password and update user profile information.
  • Trouble accessing account? Select Forgot Password to receive a link via email to reset your password or contact the system administrator.

      Price Survey

  • Enter a price for each item in the “Price Per Item” field. All fields must be completed.
  • Click Save Information to suspend work until a later time or click Submit when data entry is complete to submit your survey.
  • Once survey is complete select Print for a copy of the file for your records.

      User Management

  • Select View/Update to view or make changes to user information.
  • To change vendor status, click Administrator/State and select active/inactive.
  • Select View/Create/Update to access and edit user information.

      Vendor Management

  • Click View/Update to view or make changes to your vendor information.
  • To change vendor status, select Administrator/State and then select active/inactive.
  • Select View/Create/Update to access and edit vendor information.

      Item Management

  • To change Pharmacy/Vendor status, select Administrator/State and then select active/inactive.
  • Select View/Create/Update to access and edit Pharmacy/Vendor information.
